A Study on Macular Ectopia Angle and Distance in Cicatricial Retinopathy of Prematurity

Purpose: To examine the macular ectopia angle and distance in children with retinopathy of prematurity (ROP) at the cicatricial stage and investigate their association with the degree of cicatrix, eye position, and proliferative tissue location.
Subjects and methods: Among a total of 402 infants who were diagnosed with ROP from 2006 to 2015 at Kochi Health Sciences Center, this study was conducted on 86 eyes of 43 patients whose fundus photographs were available. Disc-fovea angle (DFA) and disc-fovea distance (DFD) were measured on fundus photographs of infants in the sitting position. In all cases, the degree of cicatrix based on the classification of cicatrix, eye position, and proliferative tissue location were examined, and the relationships between DFA and DFD and every factor were evaluated.
Results: The mean gestational age and weight at birth were 26.4±2.1 weeks and 793.0±207.7 g, respectively. The mean age at examination was 6.4±2.4 years. In terms of the degree of cicatrix, DFA values were significantly higher in the cicatricial grade 2 group than in the cicatricial grade 1 group or spontaneously healed group (p<0.01). DFD values were significantly higher in the cicatricial grade 1 and grade 2 groups than in the spontaneously healed group (p<0.001). In terms of eye position, DFA values were significantly higher in the esotropia group than in the orthophoria group (p<0.01). DFD values were significantly higher in the esotropia group than in the orthophoria (p<0.01) or exotropia groups (p<0.001). DFD values were significantly higher in the orthophoria group than in the exotropia group (p<0.01). In terms of the proliferative tissue location, DFA and DFD values were significantly higher in both the temporal and nasal proliferative tissue group than in the no proliferative tissue group (p<0.01 and p<0.001, respectively).
Conclusion: In cicatricial ROP, DFA values were significantly higher and macula deviated to inferior in the cicatricial grade 2 group compared with that in the cicatricial grade 1 group and spontaneously healed group.
