Outcome of Repeat Surgery for Concomitant Horizontal Strabismus in Patients with a History of Surgery

Purpose: To report characteristics and therapeutic outcomes of repeat surgery for concomitant horizontal strabismus in patients with a history of surgery.
Subjects and methods: One hundred and thirty-nine patients (mean age: 37.1±23.7 years) with a history of surgery and who had undergone repeat surgery at Department of Ophthalmology, Hyogo College of Midicine Hospital for concomitant strabismus during a 7-year period starting from 2008 and whose postoperative progress had been followed up for at least 1 month were included in this study. Based on the eye positions before initial and repeat surgery at our department, we classified the patients into four groups: exotropia (XT) prior to the initial surgery and esotropia (ET) prior to repeat surgery (XT/ET group), XT prior to both surgeries (XT/XT group), ET prior to both surgeries (ET/ET group), or ET prior to the initial surgery and XT prior to repeat surgery (ET/XT group). The four groups were then examined for patient characteristics and surgical outcomes.
Results: The mean±standard deviation time up to the repeat surgery at our department was 18.9±17.4 years; this time was significantly longer in the ET/XT group (29.5 years). Furthermore, 18.7% patients had a history of undergoing multiple surgeries. The postoperative eye position was significantly improved by repeat surgery (p<0.0001), and 82.7% patients showed cosmetic healing. Double vision disappeared in 89.3% of patients with preoperative double vision, and the group with XT prior to the initial surgery showed significant improvement with respect to stereoscopic vision.
Conclusions: Repeat surgery for concomitant horizontal strabismus in patients with a history of surgery had improved esthetics and was effective in eliminating double vision. Patients with XT prior to the initial surgery are expected to also show improvement in stereoscopic vision.
