Clinical Evidence and Real Clinical Practice of Wet Age-related Macular Degeneration Therapy

In recent years, medical care for wet age-related macular degeneration (wAMD) has advanced rapidly, and abundant clinical evidence regarding medical care for wAMD has been obtained in various countries, including Japan. With our focus on the treatment of wAMD, in this review we collected major fragments of clinical evidence in the treatment of wAMD and summarized them for not only the ophthalmologists who specialize in the medical care of the retina but also the general ophthalmologists who may engage in the medical care of the retina. First, these fragments of evidence were explained and reviewed by experts of AMD. The clinical evidence regarding anti-vascular endothelial growth factor (VEGF) drugs, which are recognized as a stan dard therapy of wAMD and applied widely in clinical practice, included not only clinical study results but also real-world clinical data obtained during the actual medical care. Moreover, the clinical evidence of the regimen of anti-VEGF drugs in the maintenance phase and safety evaluation was also dealt with. Furthermore, this review included explanation for the clinical evidence of treatments for polypoidal choroidal vasculopathy and retinal angiomatous proliferation, which are classified as special types of wAMD, and pachychoroid neovasculopathy, a disease entity that is attracting attention.
