Comparison of Aflibercept and Ranibizumab Therapies Using Treat-and-Extend Regimen for Retinal Angiomatous Proliferation

Purpose: To compare the 2-year outcomes of intravitreal aflibercept (IVA) and intravitreal ranibizumab (IVR) therapies using treat-and-extend (TAE) regimen for retinal angiomatous proliferation (RAP).
Subjects and methods: We studied 33 eyes with treatment-naïve RAP that could be treated with IVA or IVR for 2 years continuously. We retrospectively assessed visual acuity, central macular thickness (CMT), central choroidal thickness (CCT), area of retinal pigment epithelium (RPE) atrophy, number of injections, and complications.
Results: Fifteen and 18 eyes were treated with IVA and IVR, respectively. There were no significant differences in baseline data between the two groups. During the 2-year treatment period, visual acuity improved significantly, CMT and CCT decreased significantly, and RPE atrophy areas enlarged significantly in both groups, and there were no significant differences in the changes in any of these parameters between the two groups. The number of injections was similar, and there were no serious ocular or systemic complications in either group.
Conclusions: TAE regimens with IVA or IVR might be effective in terms of improving visual acuity and exudative changes despite RPE atrophy enlargement in eyes with RAP. There were no significant differences in the 2-year RAP outcomes between IVA and IVR.
